How do you get your own Medicare card?

When you move out of home it is important that you contact Medicare to have a card issued in your name (however, you do have to be over 15 to have your own card). When you have your own Medicare card, you can visit a doctor without the need to use your parents card.

If you are on your parents Medicare account, you will be able to have a card issued in your name.  If not, you will need to apply and provide two forms of identification (such as your birth certificate and a key card from your bank account).
